I hereby authorize Whitening Bar to charge the credit card listed above for the membership fees as per the agreed billing frequency. I understand and agree to the following terms:
1. Automatic Billing: The membership fee will be automatically charged to the card on file based on the selected billing frequency until the membership is canceled in accordance with the cancellation policy.
2. Card on File: The credit card information provided will be securely stored and used solely for the purpose of processing membership fees.
3. Cancellation Policy: Membership can be canceled any time After the initial 3 months providing written notice at least 14 days before the next billing date. Cancellations must be submitted to Whitening Bar via email at whiteningbarllc@gmail.com or in person at 5200 Monroe Street Toledo OH, 43623.
4. Changes to Payment Information: It is my responsibility to notify Whitening Bar of any changes to my credit card information. I will provide updated information in a timely manner to avoid interruption of membership services.
5. Authorization to Charge: I authorize Whitening Bar to charge the credit card on file for any membership-related fees, including but not limited to recurring membership fees, late fees, and other applicable charges as outlined in the membership agreement.
6. Acknowledgment: I acknowledge that I have read and understand the terms and conditions of the membership and the authorization to charge my credit card on file. I agree to abide by these terms and understand that my membership is subject to the terms outlined in this form and the membership agreement.
